ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-Anna-nni <br />INSPECTION DATE: -1if11g7 INSPECTOROS INITIALS: _$K_- <br />OBSERVATIONS <br />The Division conducted a pre-operational inspection of the site on 8/1/07. Present during <br />the inspection was Mr. Kenneth Morrison of Lincoln County Planning. <br />1) Site is currently an 8.5 acres 110 construction material operation. <br />2) Lincoln County submitted a conversion application to a 112 by adding 19.6 acres to <br />the existing permit on 6/28/07.Application was called complete on 7/17/07. <br />3) Site is located approximately 15 miles North East of Kraval at an approximate <br />elevation of 4920 <br />4) Operator proposes to phase mine the added acres utilizing contemporaneous <br />reclamation. <br />5) Permit conversion application sign and boundary markers for the area to be concerted <br />was in place. <br />6) Some noxious weeds were noted in the area to be amended and the current permitted <br />acres. Operator has a noxious weed control plan and Mr. Morison stated that they <br />have already applied chemicals to remove the noxious weeds from the current permit <br />area, which was evident at the time of the inspection. <br />7) There are no man made structures with in two hundred feet of the proposed or current <br />permit area. <br />8) The site was found to be as described in the conversion application. The current <br />conversion maps have also been corrected to reflect the width of the current 110 <br />permit area. <br />9) Current llo was not active at the time of the inspection. Two front end loaders were <br />noted near the active pit area at the time of the inspection. <br />10)Mining plan will be the same as with the old 110 permit. Dozer and front end loaders <br />will be utilized.
