lPage 21 <br />MINE ID # OR PROSPECTING ID N <br />INSPECTION DATE 11 /20/96 <br />M-96-076 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS WHE <br />This pre-operations inspection was conducted by Wally Erickson, from the DMG field office in Durango, 484 <br />Turner Drive, Building F, Suite 101, Durango, CO 81301, phone 1970) 247-5193. Application for the <br />Western Flagstone Quarry No. 2 was submitted to DMG Denver office on 9/27/96. Upon submittal of the <br />additional information requested in DMG 10/1/96 incompleteness letter, DMG will find the application <br />complete far the purposes of filing and a decision date determined, pursuant to Rule 1 .4.2. The Western <br />Flagstone Quarry No. 2 is located approximately 4 miles NE of Palisade, at 5220' elevation, in portions of <br />the SW 1/4 of the SW 1/4 of Section 35, T10S, R98W, 6th PM, Mesa County. Mr. Rudy Fontanari <br />accompanied the DMG inspector throughout the inspection. <br />The application describes a 9.55 acres area to be mined in 8 phases, each phase encompasses approximately <br />3/4 acre. Basalt cobbles and boulders are to be screened from material excavated from alluvial fan deposit. <br />Material processing is limited to screening and stockpiling. Mining activities are expected to proceed to 30' <br />depth even though depth of deposit has been estimated at 50'. Mining activities will maintain 3H:1 V <br />slopes. Reclamation will occur concurrently with completion of mining each phase. Topsoil will be stripped <br />from Phase 1 area and utilized to construct sediment retention basin, located at the lower most corner of <br />the permit area. Sediment retention basin will detain affected area runoff throughout life of mine. Topsoil <br />salvaged from each succeeding phase will be live hauled to preceding disturbance. Topsoil stockpiled as <br />sediment retention embankment will be redistributed over Phase 8 area upon completion of life of mine. <br />Permit area was delineated by metal T posts and rock walls. Temporary sign, required by Rule 1 .6.211 Ilbl, <br />was not observed. Permit area is located in an area previously cleared of native PJ vegetation and <br />revegetated for livestock grazing. <br />A recently constructed stock pond was located in the Phase 1 area at the south end of the permit area. <br />Pond was incised into native ground approximately 8' depth with additional capacity provided by <br />embankment constructed above ground surface. Pond was approximately 20' wide, 30' long, and 12' deep. <br />Pond inslopes were visually estimated at 5H:1 V for the east side and 1 H:1 V for the remaining sides. Material <br />excavated from the stock pond had been screened through a grizley, fines had been utilized to construct the <br />embankment, coarse material stockpiled adjacent to the grizley. No additional disturbances were observed. <br />Observed significant man made structures within 200' of the permit boundary included two overhead electric <br />lines, located at the north end of the permit area, and an irrigation ditch located at the south end of the <br />permit area. <br />Operation will utilize an existing access road as haul road. Haul road approaches the permit area from the <br />north and provides access to Interstate Hwy I-70.
