(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-i9Rnaaa <br />INSPECTION DATE: R-~~-n'~ INSPECTOROS INITIALS: aK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the Phase I, II, III AND IV areas of <br />the leach pad on 8-21-03. Present during the inspection were Mark Tidqulst and Phil Barnes of CCBV. The <br />physical inspection began around the Phase IV pregnant solution collectlon areas. <br />While in the area collected the following readings from the High and Low Volume Solution Collection <br />Transducers for Phase IV. High Volume Transducer # 1 was not functioning at the time of the inspection, the <br />pump was on, and as noted in the previous Inspection report, once all the power transfer is complete, it should <br />be functioning as designed # 2 was at 25.7 feet of hydrostatic head. Low Volume transducer # 1 was at 6.7 and <br /># 2 was at 3.2 (all readings are in inches of hydrostatic head) both pumps were on at the time of the inspection. <br />The Hlgh volume pump caissons have been raised to accommodate the first ore liff. No major problem noted. <br />Inspected the leach pad in the Phase IV area, some ponding, more like puddles of solution was noted, a D•6 <br />DOZER was reworking the pad to allow percolation, no major problem noted. According to Phil, until CCBV <br />figures out why it is ponding in small puddles on the pad, they plan to continue using the D-6 to remediate the <br />ponding of solution on the pad. <br />Inspected the Phase I AND II Pregnant solution collection areas and recorded the following readings from the <br />Hlgh and Low volume solution collection areas. <br />Phase I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS recorded at 43.7# 2 was at 46.4 and # 3 was <br />at 46.2. Low volume transducer # 1 was recorded at .26 # 2 was at ..27. Piezometer in the area was recorded at <br />50.0 feet. <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 33.1 # 2 WAS AT 27.2 AND #6 <br />WAS AT 33.0. LOW VOLUME TRANSDUCER # 1 WAS AT .16 AND # WAS AT .34 (ALL READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD) Piezometer for the pond was recorded at 31.7 feet. <br />Inspected the external pond, it was holding water from the recent storms, while in fhe are recorded the following <br />readings from the Low Volume solution collection transducers # 1 was at 8.2 and # 2 was at 11.2 readings are in <br />inches of hydrostatic head. Earth moving work in anticipation of liner deployment, below the ARD area for the <br />additional containment area, part of the area had been lined with clay, the TR SHOULD BE SUBMITTED AS SOON <br />AS Golder and associates is finished with the report. <br />Inspected the under drain area. Both the 4-inch pipe and South and North under drains were discharging at the <br />time of the inspection. The flow was being pumped back onto the leach pad. All three lower ponds were still <br />holding a lot of water; the water was being pumped back onto the pad at the time of the inspection. All the <br />lower ponds and surface water diversion structures were also holding water but were not discharging at the time <br />of fhe inspection.
