Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-~nnn_m a <br />INSPECTION DATE: n5/~eme INSPECTOR=S INITIALS: KAP <br />OBSERVATIONS <br />1 This inspection was conducted as part of the 4-year monitoring program in the Division of Minerals and <br />Geology. Present at the inspection were Kate Pickford, DMG and Jerry Carpenter, Operator. Weather at the <br />time of the inspection was cloudy, windy, and approximately SO degrees. <br />2. A sign and boundary markers are present at the site. Much of the edges of Phase i have been disturbed by <br />mining activity at the site. The Operator plans to continue mining in a southwesterly direction from the currently <br />disturbed area. At the time of the inspection, Overburden material was stockpiled adjacent to the highwalls. <br />According to Mr. Carpenter, this stockpiled material is intended to be used as backfill for the highwalls. Topsoil <br />is stockpiled along the edges of the pit. An above ground storage tank is present at the site. The base of the <br />tank is surrounded by a berm, and Mr. Carpenter indicated that a liner is present under the tank. <br />3. All activities and completed work at the time of inspection appeared to be following the guidelines set forth <br />in the mining plan for the pit.
