(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1979-002 <br />INSPECTION DATE 3-3-2005 <br />INSPECTOR'S INITIALS SSS <br />This brief inspection was conducted to verify if any corrective actions, specified to correct problems noted in the December 8, <br />2004 inspection report, had been initiated or completed. <br />No corrective actions were noted. It appeared that the site remained in the same condition as itwas on December 12, 2004. A <br />brief phone conversation with the operator on the morning of March 3, 2005 verified that corrective actions had not been taken <br />to date, but that they would have a response to DMG by this afternoon. <br />Upon return to the DMG office on the afternoon of March 3, 2005 Lafarge had in fact submitted a letter indicating that they <br />would have all corrective actions addressed by May 1, 2005 and would submit the required Financial Warranty increase. In the <br />future, Lafarge needs to take steps to bring the site into compliance within the DMG specified timeframes, which are already <br />generous. additionally, Lafarge needs to be aware that submittal of the required bond, prior to March 28, 2005, so that it can be <br />reviewed and approved by March 28, 2005 is not extended.
