ORIGiNA6 -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-t9s;n_~ee <br />INSPECTION DATE: 5/14/Od INSPECTOROS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring and a construction progress inspection <br />of the site on 5/13/04. Present during the inspection were Tim Comer and <br />Mark Tidquist. Due to heavy fog, and light sow the night before; there was <br />no grading activity in the Phase IV area, at the time of the inspection. <br />According to Mark, Poly Flex is going to be on site to deploy geomembrane <br />some time next week. Inspected the leak detection trench in place, located <br />along the North side of the Phase IVC leach pad, no major problem noted. <br />Inspected the recently clay lined area, along the North side of the phase <br />IVC pad expansion area, no major problem noted. <br />Inspected the Phases I, II, III AND IV leach pad areas and recorded the <br />following readings from the high and low volume solution collection <br />transducers. <br />Phase I High volume solution collection transducer # 1 was recorded at 42.4 <br />#2 was at 44.9 and # 3 was at 44.9. Low volume transducer #1 was recorded <br />at .23 and # 2 was at .29. Piezometer for the pond was at 43.5 feet. <br />Phase II High volume solution collection transducer # 4 was recorded at <br />39,4 # 5 was recorded at 34.7 # 6 was at 41.4,Low volume transducer # 1 was <br />at .13 and # 2 was at .60. Piezometer for the pond was at 41.3 feet. (All <br />transducer readings are in feet of hydrostatic head unless noted otherwise) <br />Inspected the external pond, it was holding a small amount of water. Low <br />volume solution collection transducer for the pond were recorded at 12.6 <br />for # 1 and 10.2 for # 2(inches of hydrostatic head) <br />Phase IV High Volume solution collection transducer # 7 was recorded at <br />25,5 # 8 was at 25.5 #9 was at 25.6 and # 10 was at 26.8. Low volume <br />solution collection transducer # 1 was at 18.5 and # 2 was at 6.1 inches of <br />hydrostatic head.
