<br />i <br />h~INE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 12-09-99 <br />(Page 2) <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS BK_ <br />The division conducted a monitoring and construction progress inspection of the Phase II and <br />II areas of the Leach pad facility on 12-09-99. Present during the inspection was, Mr. Scott <br />LEWIS of CC&V. Inspected the Phase II pregnant solution collection area. The following <br />readings were taken from the HIGH AND LOW VOLDME COLLECTION TRANSDIICERS. High volume pomp #4 <br />had 36.2 feet of head, pump #5 had 36.2 feet of head and pump # 6 had 42.5 feet of head. Low <br />volume solution collection pump # 1 had .43 feet of head and pump # 2 had .20 feet of head. <br />Flow rate at the low volume collection pump was measured at 4.0 g.p.m. Inspected the Phase <br />II toe dam which was being raised as approved in Amendment # 7.The dam has been raised by the <br />approved 7 feet. Slopes were stable and no surface failures were noted at the time of the <br />Inspection. Inspected the Phase III expansion areas. Work was continuing on the IInder Drain <br />and LDS #4 HXTHNSIONS AND REMHDIATION OF UNDERGRODND WORKINGS. It appears some underground <br />workings have been encountered during the excavation process for the under drain and LDS <br />systems. Operator requested to replace APPROX., 50 FHHT OF the 4" corrugated pipe with solid <br />4" pipe for the LDS # 4 EXTENSION. The request was granted in~the Field. The operator also <br />requested to steepen the temporary diversion for the inter phase area of Phase II and III <br />from 14 degrees to 19 degrees. Since it ie temporary and designed for the 100 year 24 hour <br />event, the request was granted in the field. Final grade work on the uncertified Phase III <br />area was continuing. It should be completed with in the next few days. The operator also <br />requested to remediate the Boylan Shaft using similar methods as the other underground <br />remediations rather than the geo grid ae approved in the Amendment. The area of Phase II <br />covered under the Partial certification of November 1999 HAS TWO LIFTS IN PLACE.
