(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~A-ivaa-ini <br />INSPECTION DATE: R/~5/nx INSPECTORS INITIALS: ~!? <br />OBSERVATIONS <br />1. This inspection was performed as part of the four-year monitoring program conducted by the Division of <br />Reclamation, Mining, and Safety. Present at the inspection was Kate Pickford, DMG. Weather at the <br />time of the inspection was cloudy, windy, and approximately 70 degrees. <br />2. A sign is posted at the entrance to the site. Disturbance at the site consists of an excavation on the east <br />side of the access road close to County Road 54. This disturbance is located in Phase 1, as proposed in <br />the permit application materials. Further disturbance consists of a stockpile area in the northwest corner <br />of the site and stockpiles are also located along County Road 59 in the western portion of the site. <br />3. The topsoil stockpile is located in the vicinity of the stockpile area. As was mentioned in the previous <br />inspection report, referring to an inspection conducted on August 15, 2001, vegetation on the topsoil <br />stockpile is inadequate to fulfill the requirements of the Rules and Regulations (See Problem 1, Page 3). <br />4. Adjacent to the topsoil storage area there is a loading ramp and a large storage tank, with a capacity <br />of approximately 10,000 gallons. The storage tank lacks a containment structure below it (See Problem <br />2, Page 3j. <br />5. Contained in the excavation near the access road is a pile of recycled asphalt which has not been <br />approved for disposal at this site (See Problem 3, Page 4). Additionally there is a 55-gallon drum lying on <br />its side in the excavation, which contains some material, but the type of material is unknown. Labeling <br />on the drum indicates it contains transfer oil. A similar barrel was mentioned in the August, 2001 <br />inspection report and that report directed the Operator to remove the barrel from the site (See Problem <br />4, Page 4). <br />6. The other conditions at the site fall within the parameters of the permit for the site. There is some <br />equipment stored on the site. Additionally, there is an excavation in the northeastern portion of the site <br />which contains approximately 600 tires in and around the pit. A bond calculation will be conducted as <br />apart of this inspection. Costs for removal of the equipment and tires will be included in the bond <br />calculation for the site. If the operator desires to have the costs of these materials deducted from the <br />resulting bond amount, these items will need to be removed and a follow up inspection will be <br />conducted by the Division to confirm the items have been removed.
