(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1947-t nR <br />INSPECTION DATE: n7J7aina INSPECTOR=S INITIALS: ~_ <br />1. A site inspection was made of the Bad Boy Stone Quarry as part of the Division's on-going 4-year inspection program. <br />The previous inspection by the Division occurred on July 27, 1998. The quarry is located 8 miles northwest of Lyons, <br />Colorado. Present during the inspection was Mr. Pablo Vasquez. <br />2. The Bad Boy Stone Quarry is a 110 Limited Impact operation, permitted to affect 6.7 acres. There are two (2) pits <br />lopted on site. Mining has been completed at the northern pit, approximately 1 acre in size. This inspector estimates 1 <br />acre-foot of finish grading required to achieve reclamation contours at the north pit. The south pit is active with quarrying <br />activities occurring on a limited scale at the time of inspection. This inspector estimates a disturbance of approximately 4 <br />acres at the south pit with 3 acre-feet of finish grading required for final reclamation. The south pit also has a highwall <br />estimated to be 12-14' in height and 160' in length. The highwall will be reduced to a 3:1 slope as specified in the <br />reclamation plan. All disturbances were noted to be within the permitted area. The quarry will ultimately be reclaimed <br />to wildlife habitat. <br />3. An entrance sign was posted and permit boundary markers were established as required by Rule 3.1.12. <br />4. The operator has posted a tots{ financial warranty of $5,300. This inspector has estimated a cost of $3,900 to <br />complete all backfill & grading, re-spreading of salvaged topsoil, and re-vegetation. There are also a number of loads of <br />miscellaneous, non-roadable mining equipment that will need to be removed during final reclamation. Based on the <br />existing and planned disturbance at the quarry, this inspector finds the financial warranty of $5,300 to be adequate to <br />complete reclamation. The Division may consider a reduction in the amount of financial warranty required upon a <br />demonstration that miscellaneous, non-roadable mining equipment has been removed from the site. <br />5.
