• IPage 21 • <br />MINE ID # OR PROSPECTING ID # M-86-148 <br />INSPECTION DATE 4-19-99 INS ECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was an inspection of the Weaselskin Pit, conducted at the request of the operator, to evalu'3te anticipated changes <br />to the highwall backfill plan and the mining plan. The operator wishes to remove the requirement to mine the site at the <br />approved final grade. He indicated that this puts too much of a restriction on the operations. <br />The operator indicated that he would approach the permittee concerning filing of a revision to the easelskin permit. The <br />revision would address changes to the mine plan that would allow a vertical highwall to exist duri g mining. The revision <br />would also address changing the final grade of the northwest highwall from 3:1 to 2:1, except fbr the slope from 5 feet <br />above the expected water level of the pond to 10 feet below that water level. <br />If the final slope revision is to be submitted, it will need to include an engineering demonst ation, from a certified <br />professional engineer, demonstrating that the slope will be stable at the revised grade. The de onstration must certify <br />that the natural gas pipeline, located at the top of the highwall, will not be adversely affected by instability of the slope. <br />The letter of credit financial warranty form, to be filled out by the permittee for the increased (bond has not yet been <br />received by the Division. A call report, from the bank issuing the letter of credit, must also be submitted. These Items <br />must be received at the Division office, by May 3, 1999, which is the due date for having the revis d bond in place. Failure <br />to meet this due date may result in an enforcement action. <br />If the revisions mentioned above are submitted prior to the May 3, 1999 due date, the Division m~y grant an extension of <br />the financial warranty due date. <br />I & E Contact Address <br />NAME B.
