ORIGINAL -PUBLIC FILE
(Page 2)
MINE ID # OR PROSPECTING ID #: M_1999_nno
INSPECTION DATE: ~!1/nd INSPECTOROS INITIALS: ~_
OBSERVATIONS
The Division conducted a monitoring inspection of the site at the request
of the operator. The operator had submitted a technical revision to change
the sequence of mining from the Northwest to the North East side and wanted
to meet on site so they can explain the sequence. Present during the
inspection was Mr. Dave Tiptton of Gillette sand and gravel. Since the
financial warranty in place is more than adequate to cover the phased
mining affected areas, the Division will not object the mining sequence
change. Topsoil removal had taken place along the north east side of the
permit area. Topsoil piles will be protected from erosion through seeding,
once the stripping is completed. Existing topsoil piles were stable. A few
mining related equipment such as a screen and crusher were located along
the North side of the permit area. Permit sign and property markers
properly displayed. The TR will be approved; no additional financial
warranty will be needed at this time. Operator has also obtained from the
Army Corps of engineer, a temporary crossing permit of the perennial
stream, during the mining operation.
I & E Contact Address
NAME: MARK YEVOLI
OPERATOR: GILLETTE SAND AND GRAVEL
STREET: 19185 N.
