<br />Inspection Narrative <br />Permit: C-81-026 <br />Canadian Strip <br />Company: Slurco Corporation <br />P.O. BoX 281304 <br />Lakewood, CO 80228-8304 <br />Date: September 27, 1994 <br />Weather: Clear warm and sunny <br />This is a reclaimed mine site. Reclamation was accomplished <br />between 1979 and 1981 on 29.66 acres. Phase I bond release <br />has been granted on the entire site and Phase II on all but <br />55.13 acres. The loadout area in the town of Walden has been <br />totally released. Ponds #1 and #2 have been approved to be <br />left as permanent impoundments. The total bond remaining is <br />$235,182 and the permit is valid until December 1, 1996. <br />Permit documents were reviewed at the County Clerk's office <br />and the following discrepancies were noted: <br />There was no copy of the actual permit document on file. <br />There was no copy of the State's Bond Release Findings on <br />file. <br />Copies of mine site inspection reports and enforcement <br />actions were not on file and no notice to the public of <br />how or where this information could be obtained. <br />No current insurance certificate. There was one at the <br />State office. <br />State Notice of Violation C-94-026 was issued to address these <br />records violations. Other discrepancies were: <br />The second quarter NPDES surface water monitoring reports <br />were not available. <br />The Permit states that the annual Hydrology Report is to be <br />submitted by October 31st of each year but the report for <br />1993 was not submitted until January 1994. <br />State NOV C-94-025 was issued to address these violations. <br />No annual pond certifications. Quarterly pond inspection <br />reports were available but can not considered as <br />certifications since they were not signed by a Certified <br />Professional Engineer. <br />State NOV C-94-027 was issued to address this violation. <br />3treN <br />
