MINE ID # OR PROSPECTING ID #: M-~ a8n-~aa <br />INSPECTION DATE: s-~s-n~ INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the site on 5-15-02. Presents <br />during the inspection were Mark Tidiquist and Phil Barnes from CC&V and Bob Nickels, from Gotder and <br />associates, the QA/QC ENGINEERING FIRM FORTHE SITE. <br />Physical Inspection began around the Phase IV, area. Clay liner along the <br />East Side was being deployed and compacted at the time of the inspection. <br />No problem noted. Back fill and grading work on the Phase IV (B) AND (C) <br />ARES WAS CONTINUING, NO PROBLEM NOTED. <br />Continued the inspection to the new Load Out Bin and crusher area. The <br />required thickness of drain cover along the LOB area was already in place, <br />the entire area was walked and inspected, no major problem noted. work on <br />the new crusher appears to be progressing, no problem noted. Erosion <br />control structures around the facility appeared to be in good repair. <br />Inspected the Phase II pregnant solution collection area and recorded the <br />following readings from the High and low volume solution collection <br />transducers. <br />High Volume solution collection Transducer # 4 was recorded at 24.3 <br /># 5 was recorded at 30.8 <br /># 6 was recorded at 36.8 <br />Low Volume solution collection Transducer #1 was recorded at .13 <br />#2 was recorded at .14 <br />ALL READINGS ARE IN HYDROSATATIC FEET OF HEAD. <br />Inspected the underdrain area. Both the South and 4 inch pipes were <br />discharging. Tha North underdrain was still dry. No problem noted. Both the <br />lime-lined pond and the lower pond were holding some water, no problem <br />noted.
