MINE ID # OR PROSPECTING ID #: M-~ a8n-~aa INSPECTION DATE: s-~s-n~ INSPECTORS INITIALS: RK OBSERVATIONS The Division conducted a monitoring and construction progress inspection of the site on 5-15-02. Presents during the inspection were Mark Tidiquist and Phil Barnes from CC&V and Bob Nickels, from Gotder and associates, the QA/QC ENGINEERING FIRM FORTHE SITE. Physical Inspection began around the Phase IV, area. Clay liner along the East Side was being deployed and compacted at the time of the inspection. No problem noted. Back fill and grading work on the Phase IV (B) AND (C) ARES WAS CONTINUING, NO PROBLEM NOTED. Continued the inspection to the new Load Out Bin and crusher area. The required thickness of drain cover along the LOB area was already in place, the entire area was walked and inspected, no major problem noted. work on the new crusher appears to be progressing, no problem noted. Erosion control structures around the facility appeared to be in good repair. Inspected the Phase II pregnant solution collection area and recorded the following readings from the High and low volume solution collection transducers. High Volume solution collection Transducer # 4 was recorded at 24.3 # 5 was recorded at 30.8 # 6 was recorded at 36.8 Low Volume solution collection Transducer #1 was recorded at .13 #2 was recorded at .14 ALL READINGS ARE IN HYDROSATATIC FEET OF HEAD. Inspected the underdrain area. Both the South and 4 inch pipes were discharging. Tha North underdrain was still dry. No problem noted. Both the lime-lined pond and the lower pond were holding some water, no problem noted. According to Phil, CC&V plans to do some maintenance work on the lime lined work and also add some more lime, in the next weak or so, no major problem noted.
