Iuppa, landowner and Energy Fuels caretaker <br />This was a complete inspection of a reclaimed mine. Parts of the mine have a Phase I <br />release and parts have a Phase III release; all of the disturbed area has been reclaimed. <br />The permit expires 8/17/2002. 1310 acres are permitted and 30.6 acres are disturbed. <br />The NPDES # is COG850000. Remaining bond is $205,831. The liability insurance is <br />for $2M with American Home Assurance. <br />There has been a partial Phase I bond release for 25.3 acres with the facilities, water tank, <br />etc., being excluded. Sante Fe loadouts 1 and 2 have received a Phase III release. There <br />is an industrial post mining land use and the city uses it for sludge storage. <br />The pond has an as-built certification dated 1/14/94. It consists of a pipe and open <br />channel spillway. The last quarterly inspection for the waste pile is dated 4/15/00. <br />DMR's are current through the first quarter of 2000. Monthly ground water monitoring <br />is up to date. <br />The topsoil stockpile has an approved SAE. There is a fair amount of sheet erosion on <br />the other area. The ditch SAE is vegetated and rocked and is preventing erosion gullies. <br />
