(Page 2)
MINE ID # OR PROSPECTING ID #: M-i9nR-~aa
INSPECTION DATE: a-~a-m INSPECTORS INITIALS: RK
The Division conducted a monitoring inspection of the site on 8-16-01. Present during the inspection was
Mr. Phil Barnes from CCB:V. The area had received measurable precipitation the past few days and the main
purpose of the inspection was to make sure the storm water diversion structures around the leach pad
facility were functioning as designed. The division channels around the leach pad facility were driven and
inspected all appeared to have functioned as designed and were in good repair. Topsoil stripping and
grading of the Phase IV area was continuing, no problem noted. Continued the inspection to the Phase II
pregnant solution collection area and recorded the following readings from the high and low volume
solution collection transducers. High volume pump # 4 was recorded at 8.6 feet of head, # 5 was at 11.7
feet of head and # 6 was at 13.9 feet of head. Low volume solution collection pump # 1 was at .07 feet of
head and # 2 was at .19 feet of head.
Inspected the toe dam area, all three ponds were holding and discharging at the time of the inspection.
Water was being pumped from the lower pond to the external pond at the time of the inspection. All three
underdrain discharge pipes were also discharging at the time of the inspection but no samples were
collected.
Highway relocation bridgework was continuing, no major problem noted. Inspected the external pond area,
fence post around the pond was being placed at the time of the inspection. As mentioned above, water from
the lower Arequa pond was being pumped to the pond for temporary storage.
Inspected the new crusher area, sheet piling instead of soil nail wall was being placed at the time of the
inspection. -The original design called for a soil nail wall but according CC&V, the sheet piling appears to
give better support than the soil nail design.
With Mr. John Hardaway present, the Moffat tunnel area was inspected. Water from the base of the cribbing
was discharging to the county road. The source of the water appeared to be the natural drainage to the
west of the portal that infiltrates to the cribbing and discharging at the base of the cribbing. No discharge
from the portal opening was noted.
I & E Contact Address ^ OTHER
NAME: SCOTT LEWIS
OPERATOR: CC&V
STREET: P.O.BOX 191
CITY/STATE/ZIP: VICTOR, COLORADO R0R6n
JAMES STEVENS
cc ^ CE
^ BL
^ FS
^ HW
^ HMWMD (CH)
^ SE
^ WQCD (CH)
