• (Page 21 <br />• <br />MINE ID ~ OR PROSPECTING ID ~ M-92-060 <br />INSPECTION DATE 11 /18/99 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as follow up to verify the adequacy of the corrective actions performed by <br />the operator as a response to problems noted at this 110 permit. The operator was contacted about the inspection but <br />was not able to be present for the inspection. <br />The operator was to perform specific corrective actions by a deadline date, which had been extended to 10/1 /99. On <br />9/16/99 the operator notified the Division by letter that the corrections were completed, and the Division considered the <br />problems to be abated. This inspection was to verify that the corrections are, in fact, adequate. The following <br />observations relate to each specific task. <br />Corrective action ~1: The operator was required to post a permit ID sign and mark the permit boundaries, pursuant to Rule <br />3.1.12. The ID sign was observed posted at the entrance to the property, at the intersection of Henderson Rd and the <br />access road to the site. This location is appropriate, though (in view of an apparent misunderstanding explained in the next <br />paragraph) it is noted here that this location is not at the permit boundary. <br />The permit boundarv markers were not observed in the orooer locations. This permit area is for just 6.0 acres, and does <br />not include that area adjoining Henderson Rd. Upon leaving the site, two orange-painted posts were observed along the <br />east sled of Henderson Rd., which are presumed by this inspector to be two of the four recently-installed boundary markers <br />(incorrectly marking the NW and SW cornersl• The two other markers (for the NE and SE corners) were not observed. The <br />original problem of lack of boundary markers is still considered to be abated, but the operator must now install the four <br />markers in the orooer locations. If there are questions, please contact this office or consult the permit map. Upon orooer <br />permit boundarv/corner marking, please submit adequate proof to this office of such installation. <br />Corrective trction Jt2: The operator was to provide a Notice of material generated offsite and being imported for backfill„ <br />pursuant to Rule 3.1.5(91• An adequate description of the material was submitted, and the problem is considered to be <br />abated. This problem will not recur as long as all future imported material is of the same tvoe backfilled the same wav <br />and the area reclaimed the same wav. <br />Corrective action #3: The operator was required to submit an adequate revegetation plan, since none was ever provided <br />at the time of permit application or issuance. The operator provided such a plan, prepared in consultation with the USDA <br />NRCS. The plan is adequate, and has been added to the requirements of the approved reclamation plan. This problem is <br />considered to be abated. <br />Other observations at the site during the inspection include: all stockpiled material has been removed, leaving a level pad <br />in the center of the permit area. Backfilling is ongoing, along the north edge of the slough which crosses the site; new <br />material consists of earth and concrete rubble, and the newly filled areas are greded smooth. No revegetation has occurred <br />yet, though some control of annual weeds Iby burning) has recently been performed. <br />There are no further observations, and no problems at this time.
