ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-teRnand <br />INSPECTION DATE: o-I ~-0'~ INSPECTOROS INITIALS: fit` <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the Phase 1, II, III AND IV areas of <br />the leach pad and water sampling from the North Under drain Discharge, on 9-17-03. Present during the <br />inspection were, Tim Comer and Mark Tdquist of CC&V. The physical inspection began around the Phase IV <br />pregnant solution collection areas. Prior to the inspection Mr. Tim Comer informed me of an accidental fear to <br />the liner near the temporary power source area, where the housing for the transducer read out will be placed, <br />during the during the transportation and placement of the primary header pipes. He stated that no one knew <br />how it happened but it will be repaired as soon as Poly Flex gets back to the site and the certification report will <br />reflect the proper documentation of the repair work. Only drain cover was being pushed along the North sides <br />of the pad area, at the time of the inspection. The tear area was observed, the operator had placed a liner <br />above it to prevent rainwater from seeping. No major problem noted. The permanent pumps have been placed <br />on the caissons. Once the power is connected and the permanent pumps are connected the main power <br />source, the transducer readouts for the High and Low volume solution collection systems will also be in place <br />and housed. <br />While in the area collected the following readings from the High and Low Volume Solution Collection <br />Transducers for Phase IV. High Volume Transducer # 1 read out was not functioning at the time of the inspection, <br />the pump was on, and as noted in the previous inspection reports. # 2 was at 18.2 feet of hydrostatic head. Low <br />Volume transducer # 1 was at 5.9 and # 2 was at 1.3 (all readings are in inches of hydrostatic head) both pumps <br />were on at the time of the inspection. Inspected the recently deployed ore, the second liff was being placed at <br />the time of the inspection. No ponded solution was noted. <br />Inspected the Phase I AND II Pregnant solution collection areas and recorded the following readings from the <br />High and Low volume solution collection areas. <br />Phase I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS recorded at 40.5# 2 was at 43.2 and # 3 was <br />at 43.9. Low volume transducer # 1 was recorded at .22 # 2 was at .71. Piezometer was at 46.9 <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 34.0 # 2 WAS AT 28.0 AND #6 <br />WAS AT 34.0 LOW VOLUME TRANSDUCER # 1 WAS AT .16 AND # WAS AT .51 (ALL READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD) Piezometer for the pond was recorded at 34.8 feet. <br />Inspected the external pond, it was holding water from the recent storms, while in the are recorded the following <br />readings from the Low Volume solution collection transducers # 1 was at 11.7 and # 2 was at 13.2 readings are in <br />inches of hydrostatic head. Earth moving work in anticipation of liner DEPLOYMENT near the ARD building for <br />additional solution containment, IS CONTINUING. Once Poly Flex returns to the site, CCBV expects to have the <br />LINER WORK COMPLETED. Trenches were being dug to remove some buried gas and water pipes. No major <br />problem noted. <br />Inspected the under drain area. Both the 4-inch pipe, South and North under drains were discharging at the <br />time of the inspection. The flow was being pumped back onto the leach pad. The lower pond was being <br />mucked out for additional storage at the time of the inspection.
