<br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-~aRn-~aa <br />INSPECTION DATE: ~aF-nn <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS: ~_ <br />The Division conducted a monitoring and construction progress inspection of the Phase II and III areas of the leach pad <br />facility. In the office, Mr Barnes explained CC&V's plan to construct a new haul road with in the affected acreage east of <br />the pad area South West of the ARD building. He was informed to include it in the TR with the appropriate map, CC&V is <br />going to submit for the upcoming construction season for the phase IV area. Present during the inspection were Mr. Phil <br />Barnes and Mr. Mark Tidquist of CC&V. Mr Tidquest is going to be the contact person for construction related activities, <br />according to Phil Barnes. The inspection began around the Phase III area, temporary storm water diversion work was <br />continuing, no major problem noted. The temporary road culvert located in the area appeared to be in good repair. In <br />order to avoid mistakes due to the activities in the area, CC&V had placed temporary stakes delineating lined area. Once <br />the activity is completed, permanent markers will be placed in that area. Inspected the Phase II pregnant solution <br />collection area and recorded the following readings from the High and Low volume solution collection transducers. High <br />volume transducer # 4 was not functioning at the time of the inspection. Mr Bames stated that CC&V was trying to <br />identify the reason why it stopped functioning, # 5 was recorded at 15.9 and # 6, read 17.2, and low volume transducer <br /># 1 was recorded at .07 and # 2 was at 06. (ALL MEASURMENTS ARE IN FEET OF HYDROSTATIC HEAD) with the <br />exception Piezometer # 1, which was recorded at 27.4 feet as measured from the top of the casing. <br />Inspected the underdrain area, work was continuing on the highway Relocation Bridge and the solution collection pond. <br />The south underdrein 24-inch pipe and the 4-inch pipe were discharging, no problem noted. Continued the inspection to <br />the phase I leach pad areas. Mr. Barnes pointed out the probable location of the new haul road near the ARD building. He <br />was informed to TALK TO Terry Madziac about towering the slopes below the lab area, which experienced cyanide spills <br />that were, contained with in the affected acreage but out side the protected area. <br />While in the area, the following readings were collected from the phase I high volume solution collection transducers and <br />piezometer. Pump # 1 has not been functioning for quite some time due to problems with the caisson, CC&V plans to <br />remove the caisson to identify the problem during the spring according to Mr. Barnes. Pump # 2 was recorded at 19.0, # <br />3 was recorded at 23.3 feet of head. Piezmeter # 1 was at 34.4 feet. <br />Continued the inspection to the Squaw gulch waste storage are where the operator has placed storm water detention <br />basins in the thalweg of the channel as approved and later amended by CC&V.
