(Page 2)
MINE ID # OR PROSPECTING ID #: M-1978-116
INSPECTION DATE: 2-7-07
INSPECTOR'S INITIALS GRM
OBSERVATIONS
This inspection was conducted as part of the normal monitoring program established by the Colorado Division of
Reclamation, Mining and Safety for permitted sites. The primary reason for the inspection was a fact finding tour by
Division Geo-Chemist David Bird. Mr. Bird is now involved in the review of uranium mines and the Designated
Mining Operation appeals on these sites. Also, it provided staff an opportunity to verify the location and installation
of pizometers at the 4 current sites under DMO review.
The mine identification sign and affected azea boundary markers aze in place and in compliance with Rule 3.1.12.
A prior inspection noted some minor erosional issues from a major precipitation event. The inspector notes that silt
fencing has been replaced with rock armor along the constructed drainage between the waste and topsoil piles. Other
erosion in this area was repaired as well.
The pizometer is located just east of the waste piles and against the rock ledge that defines the azea. One of the
accompanying pictures notes the black and blue cap typical of the installation of al] four wells. Cotter will be
submitting data from this well with the other 3 sites prior to the Board Hearing on the DMO issues.
At this time there is no activity on the site. No debris, ore stockpiles or fuel storage is noted at this time.
Mr. Bird discussed and observed the general geology and hydrology of the mine area with Mr. Williams.
No issues aze noted at this time.
Responses to this inspection report should be directed to Russ Means at the Division of Reclamation, Mining and
Safety, Grand Junction Field Office, 101 South 3`d Street, Room 301, Grand Junction, Colorado 81501, phone no.
970-241-1117.
I & E Contact Address cc:
NAME: Glen Williams
OPERATOR: Cotter Corporation
STREET: P.O.
