<br />(Page 2) <br /> <br />MINE ID f OR PROSPECTING ID ~ M-87-009 <br />INSPECTION DATE 3-9-93 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />The required entrance sign is posted at the site, however the permit boundaries are not <br />marked. Since the map presently included in the file for this pit does not accurately <br />represent the pit ae it has been developed, the operator must submit a new ma.p of the permit <br />area which shall define the permit boundaries, depict acreage presently disturbed, and chow <br />location of permit boundary markers. <br />Topsoil ie stockpiled at the north and south ends of the pit, there are eevaral sand piles <br />on the pit floor. There ie a high-wall, 10 to 15 ft. in height along the east end of the <br />pit. <br />The existing 55000.00 bond held for this site is inadequate to cover the coat of reclamation. <br />The following is a reclamation cost summary: <br />TASK NO. TASK DESCRIPTION COST(S1 <br />1 Mobilize and de-mobilize dozer needed for reclamation (see $439.07 <br />attached worksheet).
