ORIGINAL -PUBLIC FILE (Page 2) MINE ID # OR PROSPECTING ID #: M-~ 9an_~d9 INSPECTION DATE: IO/n~/ns INSPECTOROS INITIALS: ~- OBSERVATION The Division conducted a monitoring inspection of the site on 10/07/05. The site is a very small 1 10 sand and Gravel operation located along State High way 24 near Green Mountain Falls Colorado. Present during the inspection was Mr. Charles Houchin the operator. The site was not active at the item of the inspection but fresh cuts were noted on the active slope face, which indicates recent mining activity. On August 15, 2005, the Division approved a Technical Revision to change the post mining slopes from 3:1 to 2:1. Active mining face was walked and inspected, slopes were stable and no major sign of erosion than what would be expected for such cut slopes were noted. In the last inspection the Division ahead discussed about the close proximity of mining to a power pole was mentioned along the East side of the permit area and Mr. Houchin had stated that the Power Company was planning to come and move the power pole. As he stated the pole had been moved, however since the mining is still with in two hundred feet, the Division requests the operator to provide the written agreement with the power Company for our files, that allows them to mine with in less that 200 feet of man made structures in lieu of an engineering study. Since the power company had moved the pole and are aware of the close proximity of the mining operation to the pole. As in the previous inspection report the fact that mining is with in 20o feet will not be sited as a PB (POSSIBLE PROBLEM) because the above reason. Permit sign and boundary markers were properly posted. The financial warranty for the site is adequate to cover the cost of recalamtion. With the recent change in post mining slopes, operator will have to move less material to achieve the desired slope. Operation is in compliance with the approved reclamation plan.
