• (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-BO-244 <br />INSPECTION DATE 8/2223 /94 INSPECTOR'S INITIALS JAP <br />Following the close-out conference I departed the mine site and registered at the Victor <br />Hotel. At 8:20 pm, following sunset, I returned to the overlook on the county road, from <br />which I could observe activity throughout the majority of the Phase 1 pad area from road <br />shoulder, within the right-of-way. The purpose of my observation was to determine whether <br />any work was being conducted on the soil liner or geomembrane liner. Ames equipment was <br />concentrating upon continued excavation of the high areas of the south facing pad elope. <br />Working lights were illuminating the area being excavated. No lights were illuminating the <br />north facing elope of the pad. However, sufficient defused construction lighting and moon <br />light existed for me to determine that no work was being conducted on the soil liner or <br />geomembrane liner system. <br />I returned to the site at 7:15 am with the intention of observing the Contractor's <br />performance, specifically in regard to the accordance with cold weather performance <br />criterion. While the weather forecasters had predicted sub-freezing temperatures, my <br />thermometer indicated a temperature of 3S°F at the overlook from the County road above the <br />Phase 1 pad. Gundle was beginning to deploy geomembrane liner on the area of soil liner <br />which had been previously prepared and pronounced acceptable by Ames. At 7:30 am Terry <br />Manziac, driving by on the county road stopped and joined me. We then proceeded to the CC6V <br />office where I informed John Hardaway of my presence. <br />John Hardaway accompanied me to the area of the geomembrane liner deployment. I observed <br />three panels of geomembrane being deployed. Prior to the deployment of the third panel a <br />soft area of soil liner was detected by observing kneading beneath the Ames drum compactor. <br />It appeared that the soft area was previously the temporary diversion trench installed to <br />protect lower soil liner from rvnon of precipitation off the previously installed geomembrane <br />liner deployed upelope. After three passes with the compactor, the zone stiffened to the <br />point where it was no longer kneading sufficiently to crack the compacted surface. The <br />geomembrane liner was then deployed. After consultation between Ames and Gundle personnel <br />it was decided to continue geomembrane deployment in the area at the western terminus of the <br />north facing pad elope, adjoining and above the south end of the Phase 1 toe berm. <br />Mr. Hardaway and i next toured the site of the primary and secondary crushers. The concrete <br />structure of the primary crusher ie completed and the approximately BO foot high Hilficker <br />retaining wall ie nearing completion. The structure for the two secondary crushers are also <br />nearly completed. The steel superstructure and mechanical components of the majority of the <br />crushers and screens are laid down in the area adjoining the crusher installation. <br />I completed my inspection by accompanying Mr. Hardaway to the site of the upland diversion <br />box culvert. The installation of the box culvert has been delayed awaiting cultural and <br />antiquities clearance from the USBLM for the area required for the temporary relocation of <br />the roadway. The culvert for the road relocation is already oneite and in place. Mr. <br />Hardaway projects one month to complete the box culvert, which is required to complete the <br />upland diversion for the Phase 1 leach pad. <br />During our second close-out conference, i raised the topic of ore placement on the Phase 1 <br />leach pad. Mr. Hardaway's recent winterization proposal had mentioned the potential <br />placement of ore on the partially completed pad. I reminded John that the Division would <br />need to receive, review and accept the final certification report, including as-built <br />drawings, prior to the placement of any ore, other than the 3 foot armor layer, on the pad. <br />Submittal of reports for discrete portions of the pad would be acceptable. Mr. Hardaway <br />differed with my opinion, stating that he believed the permit required the completion of <br />certification prior to application of processing solution, but did not disallow the placement <br />of ore.
