ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: <br />INSPECTION DATE: ii(~~- l ~1~ ~_ INSPECTOROS INITIALS: _BK__ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 1/5/04. <br />Present during the inspection were Tim Comer and Phil Barnes of CC&V. The <br />main purpose of the inspection was to collect winter quarter water samples <br />from the under drain discharge points. Agproximately 4-6 inches of snow was <br />noted on the ground from snowfalls the previous two days. <br />Inspected the Phase I, II, III AND IV pregnant solution collection areas <br />and recorded the following readings from the high and low volume solution <br />collection transducers. <br />Phase I HIGH VOLUME SLUTION COLLECTION TRANSDUCER # 1 WAS RECORDED AT 38.9 <br /># 2 WAS AT 41.8 AND # 3 WAS AT 38.0. LOW VOLUME SOLUTION COLLECTION <br />TARNSDUCER # 1 WAS RECORDED AT .22 AND # 2 WAS AT .58. <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRABSDUCER # 4 WAS RECORDED AT <br />31.5 # 5 WAS AT 33.1 AND # 6 WAS AT 32.4. LOW VOLUME TRANSDUCER # 1 WAS AT <br />.18 AND # 2 WAS AT .52 (ALL READINGS ARE IN FEET OF HYDROSTATIC HEAD UNLESS <br />NOTED) <br />PHASE IV HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 7 WAS RECORDED AT <br />13.1 # 8 WAS AT 13.2 #9 WAS AT 13.2 AND # 10 WAS AT 13.6. <br />LOW VOLUME # 1 WAS AT 21.5 AND # 2 WAS AT 21.5 INCHES OF HYDRSTATIC HEAD. <br />CC&V had installed a three horse power pump in place of the smaller pump as <br />part of the remediation plan, so the low volume solution will not exceed <br />the 24 inches of head in the double lined area of the pond. <br />With Mr. Lawrence Meyers present, inspected the AG 1.5, it was dry. <br />Inspected the under drain area, both the south and 4-inch pipes were <br />discharging and collected two 8 ounce samples using CDOH&E PREPARED <br />BOTTLES. Operator was offered a split but decided to use their own bottles <br />to collect the samples.
