<br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1447-OSR <br />INSPECTIOIJ DATE: 4-F-n1 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS: ~L~ <br />The Division conducted a joint monitoring inspection of the site with representatives from BLM. Operator <br />was notified of the pending inspection but was unable to attend. Present from BLM during the inspection <br />were Mr. Dan Grenard, Dr Charlie Fair and Miss Eadie Blanmanship. The site is a small 110 (2) mining <br />operation located in Fremont County, approximately 2.5 miles East OF Howard, CO. <br />Site was not active at the time of the inspection. <br />The Division approved reclamation plan for the site only requires the operator to reclaim the processing and <br />stockpiling areas. These areas are to be reclaimed with 6 to 8 inches of topsoil after final grade, which will <br />not exceed 3:1. <br />The required reclamation for the pit area as approved by the Division only requires the operator is to slope <br />the high wall to a 2:1 SLOPE SO IT RESEMBLES THE NATURAL CONTOUR, AND PIT LOORS TO A 3:1 SLOPE. <br />No additional reclamation is required of the operator; No Road reclamation is required according to the <br />Division Permit. <br />However, according to the special stipulations submitted by BLM as part of Exhibit "B", in the original <br />permit application, upon completion of operations, the access road between the quarry and the processing/ <br />stockpile area, is supposed to be reclaimed according to the following requirements. <br />A) All bermed material will be pulled back onto and evenly and distributed on the roadway. <br />B) The roadway will be ripped to reduce compaction. <br />C) Roadway will be contoured to approximate original contour. (drainage established to natural <br />configuration ) <br />D) Water bars will be constructed to BLM standards. <br />E) Roadway will be seeded in accordance with SCS seeding requirements. <br />A small amount of processed material was located along the East Side of the permit area. The mine consists <br />of a few benches measuring approximately 20-30 feet in height. Benches stable no major problem noted. <br />According to BLM representatives present on site, they had discussed changing the reclamation plan to <br />include day lighting the pit area, which will require removing a large quantity of material. Since the <br />approved plan does not call for day lighting the pit, if BLM and the operator agree, the Division would allow <br />the operator to submit a TR (Technical Revision) to modify the reclamation plan and post the appropriate <br />warranty with BLM. Once the appropriate warranty is posted with BLM, the Division will keep a token bond <br />and release the existing warranty. <br />I & E Contact Address <br />NAME: MR.
