(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-t 9A1-t t 6 <br />INSPECTION DATE: 9-77-n1 INSPECTOR'S INITIALS: ~_ <br />OBSERVATIONS <br />Completed a partial inspection of this site as part of a Mined Land Reclamation Board directive to inspect every mine in the <br />state at least once every four years. The site was last inspected June 16, 1998. <br />This is a 9.2 acre Limited Impact permit located about 7 miles northeast of Kornman (0.8 miles north of Prowers county <br />road VV on Prowers county road 8, then east about 1 mile on a farm road, then south and east through a farm yard, then <br />east to the first gate, Then south about 11/2 miles to the site). <br />Observed a mine ID sign at the entrance on the north side of the permit area. Did not observe any permit boundary <br />markers. The permit area configuration is a circle with a radius of approximately 357 feet. The operator must accurately <br />locate the center of the circle (1420 feet south of the north section line and 1205 feet east of the half section line; section 9, <br />T21S-R46W) then delineate the circumference with permanent markers (piles of excavated material would suffice). <br />Mining has not occurred at this site for some time. If the site is to remain inactive, the operator must submit a request for <br />temporary cessation R.C.) pursuant to requirements of Rule 1.13. The first, and second, five-year terms of T.C. must be <br />approved by the Mined Land Reclamation Board. So the item would be presented to the Board at the next available Board <br />meeting after receiving the request for T.C. Make sure to provide the information in Rule 1.13.5(2) if this is the option you <br />select. The date of cessation shall begin with the start of the 180-day period of production cessation. <br />Alternatively, the operator may provide a substitute for notice of temporary cessation by submitting a technical revision, <br />along with an appropriate fee, requesting intermittent status pursuant to Rule 1.13.7. Compliance with C.R.S. 34-32.5- <br />103(11)(6) is a must if applying for intermittent status (see Rule 6.3.3(1)(a) also). <br />The third option would be to reactivate mining activities at this site by, or soon after, the December 3, 2001 due date. <br />Notify the DMG by mail by December 3rd if the operator intends to reactivate mining activities. Provide specific dates. <br />Two problems are noted on page 3 of this report.
