• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-79-206 <br />INSPECTION DATE 3/9/99 INSPECTOR'S INITIALS AJW <br />OBSERVATIONS <br />This was a partial warranty release inspection conducted by Tony Waldron of DMG along with <br />J.E. Lionelle of U.S. Soil Conditioners. The site is located approximately 7 miles southeast <br />of Salida, Colorado and just north of U.S. Hwy 50. <br />The operator requested a partial financial warranty release to delete the Wellsville <br />Processing Plant out of their mining and reclamation permit. This plant has been there since <br />1933 and was originally used to crush limestone and is currently used as a custom crushiny, <br />processing and bagging facility for various rock products as well as by U.S. Soil for their <br />processing purposes. The actual mine site is located 2-3 miles north of the plant and is a <br />quarry type operation. <br />During the original permitting, it was believed that this plant would be used only by U.S. <br />Soil to process material from the Maverick Placer for their product line. However, since the <br />Wellsville Plant is now used to commercially process material from other mines in the area, <br />it rs no longer considered a "captured plant" used only to process mined material from the <br />Maverick Placer. As a result, the request to delete the plant from this permit will be <br />approved and the current financial warranty of $32,500.00, in the form of Irrevocable Letter <br />of Credit No. 103, will be retained to cover the reclamation liability for the mine site. <br />I & E Contact Address <br />NAME J.E.
