(Page 3) <br />MINE ID # OR PROSPECTING ID # M-1977-568 <br />INSPECTION DATE 3/21/06 <br />INSPECTOR'S INITIALS RCO <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The operator stores diesel fuel without impermeably lined containment, and has spilled 7/31/06 <br />fuel during past handling, contaminating the ground onsite. PB or PV: PB <br />TOPIC(S): AT <br />CORRECTIVE ACTION: The operator must excavate and decontaminate the ground _ _, <br />there (by offsite disposal or onsite weathering of the soil, as described in the report), <br />and place all fuels and other fluids stored onsite in an adequately-sized and lined facility, <br />which also catches spills. Evidence of completing these activities must be received by <br />this office by the date shown at right. <br />2. The topsoil is not being protected adequately, though the approved mining and 7/31/06 <br />reclamation plans require that all topsoil is salvaged for reclamation. PB or PV: PB <br />TOPIC(S): TS <br />CORRECTIVE ACTION: The operator must begin pulling back all topsoil that is within _ _, <br />12 feet of the top of the highwall (12 ft is the approx. the reach distance of a backhoe) <br />for short-term protection, and properly store such soil. The operator must also submit a <br />commitment to long-term soil protection, which must include salvage of all available <br />topsoil in areas to be mined prior to disturbance, and adequate topsoil storage. <br />Evidence of the short-term salvage and the commitment to long-term protection must <br />be received by this office by the date shown at right. <br />3.
