ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_IVRn_~ee <br />INSPECTION DATE: 8lOZ/~5 INSPECTORCS INITIALS: ~- <br />OBSERVATION <br />The Division conducted a monthly monitoring inspection of the site on <br />9/6/05. The area had received measurable precipitation the night before, <br />which was evident with small ponded water through out the permit area. The <br />main purpose of the inspection was to determine a new seismic monitoring <br />location for the flower shop monitor that was being affected by <br />reconstruction work. As approved by the MLRD Board in August 2000 under <br />Amendment #S, two seismic monitors were placed in the city of Victor, one <br />in the City Hall building and one in the flower shop. The owner of the <br />flower shop is upgrading the building, which necessitated the location <br />change. With Gary Horton, the person in charge of collecting the data from <br />the monitors present, an appropriate location away from the construction <br />area along the North side of the building was agreed upon for the new <br />location. The mine inspection began at the East Cresson mine Expansion <br />area. The pit was active with two shovels actively removing ore. <br />Backfilling of the second lift of the already mined pit was continuing. Pit <br />walls appeared stable. Storm water management structures in the area were <br />found to be in good repair. The Portland pit recalamtion South of the East <br />Cresson mine is continuing, with topsoil application in progress. <br />Continued the inspection to the Iron Clad and Globe hill areas. Some of the <br />large used tires have been removed from site to an approved disposal area <br />in Wyoming according to Mr. Goodrich. Inspected the Main and West Cresson <br />areas, both were active at the time of the inspection. During the <br />inspection, CCTV mentioned the fact that they would like to place another <br />tank of cyanide near the ARD building in a double lined area, for storage <br />at the request of the manufacturer, to make sure the site has enough <br />cyanide in the event they could not deliver due to some anticipated <br />disruption in the delivery of the product. Since the area where it will be <br />placed is near the existing tank in a protected area, the Division does not <br />have a problem. However, CC&V was informed to update the total toxic <br />chemical inventory section of the application, which they agreed to submit <br />once the other tank was delivered. Storm water management structures in the <br />area were found to be in good repair. <br />Inspected the Phase I, II, III AND IV High and Low Volume Solution <br />collection areas and recorded the following readings from transducers. <br />Phase I High Volume solution Collection Transducer # 1 read out gauge was <br />still not functioning, as noted in the earlier inspection reports, it will <br />be replaced once the pump repair or replacement is needed # 2 was at 54.6 <br />and #3 was at 55.3. Low volume # 1 was recorded at .23 and # 2 was at <br />1.5.Piezometer for the pond was measured at 53.6 feet. Phase II AND III <br />High Volume solution collection transducer #4 was recorded at 40.3 #5 was <br />at 37.4 and # 6 was at 43.5. Low volume solution collection transducer # 1 <br />was at .15 and # 2 was at .39. Phase IV high volume solution collection <br />transducer # 7 was recorded at 40.2 # $ was at 39.1 # 9 was at 41.0 and # <br />10 was at 41.2.Low volume solution collection transducer # 1 was recorded <br />
