(Page 2)
MINE ID # OR PROSPECTING ID #: ~+-teea_ns9
INSPECTION DATE: n9/t 5/nd INSPECTOR'S INITIALS: KAP
OBSERVATIONS
1. This inspection was conducted as part of the 4-year monitoring program in the Division of Minerals and
Geology. Present at the inspection were Kate Pickford, DMG, and Laurine Koellner and Jason Frost, L & L Ready
Mix. Weather at the time of inspection was clear, breery, and approximptely 65 degrees.
2. A sign was posted at the entrance to the site and boundary markers were in place. The site is currently
being mined with reclamation concurrent with mining activity. Much of the disturbed area in the Eastern
portion of the site has been graded, seeded, and vegetation is becoming established. Slopes on the
reclaimed portions of the site are 3H:1 V or less. Some of the disturbed area is still being mined and that area
contains a highwall. There was standing water in the base of the pit at the time of the inspection, due to
recent precipitation.
3. Ms. Koellner stated that they would probably continue mining to the east instead of to the west, as was
stated in the approved Mining Plan. The area she proposes fo continue mining in is still within the approved
permit boundary. She was advised that a Technical Revision must be submitted to the Division for review and
approval prior to deviating from the current plan.
4. All conditions and activity at the time of this inspection were within the limits of the approved permit,
I & E Contact Address
NAME: I aurine Koellner
OPERATOR: I R I Randy Mix
STREET: .l? f~._Bax218
CITY/STATE/ZIP: Hnvtnq,r('~, 8073]
Cc ^ CE
^ BL
^ FS
^ HW
^ HMWMD (CH)
^ SE
^ WOCD (CH)
^ OTHER: Cart Mount,DMG
