(Page 2)
MINE ID # OR PROSPECTING ID # M-1986-016
INSPECTION DATE 11/30/2006 INSPECTOR'S INITIALS DEG
OBSERVATIONS
This inspection of Permit Number M-1986-016 (Caldwell Brothers 3) was conducted by Deb E. Gbnima of the DRMS as part of
the routine monitoring and inspection protocol. No company representative was present during this inspection. The pit is
located 3 miles south of Rocky Ford, CO, and currently consists of approximately 51 acres.
The mine entrance identification sign must be posted per Rule 3.1.12(1)(c). It appeared that current disturbance was localized
within the approved permit area. Please note if the permit boundary is the fenceline running the perimeter of the northern end of
the property, then that must be clarified with photos to be submitted to the Division on or before March 1, 2007. Per Rule
3.1.12(2) the operator must clearly mark all boundaries of the affected area with monuments or other markers that are clearly
visible and adequate (the fenceline will suffice) to delineate such boundaries (see Problem No. 1 and the corresponding
corrective action noted on page 3 of this report).
All other activities and completed work at the time of inspection appeared to follow the guidelines and regulations set forth by
the state and the approved mining and reclamation plans for the pit, and no problems are noted for this site.
There was no equipment on site at the time of this inspection, and no evidence of fuel or hazardous materials was observed.
The site appears stable, and no disturbance was observed outside of the approved permit boundaries. For future reference,
this site is correctly positioned in the DRMS online GIS.
Responses to this inspection report should be directed to Deb E. Gbnima at the Division of Reclamation, Mining and Safety,
Denver Office, 1313 Sherman Street, Room 215, Denver, Colorado 80203, phone number 303-866-4179.
I & E Contact Address
NAME
OPERATOR
STREET
Wayne Caldwell
Caldwell Brothers
25026 Rd. 19
cc: ^ CE
^ BL
^ FS
^ HW
^ HMWMD (CH)
^ SE
^ WOCD (CH)
^ OTHER
CITY/STATE/ZIP Rockv Ford.
