(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-ie~~-nroi i~ <br />INSPECTION DATE: s-end INSPECTORS INITIALS: in <br />OBSERVATIONS <br />Completed an inspection of this operation as part of a Mined Land Reclamation Board directive to inspect <br />every mine in the state at least once every four years. The site was last Inspected June, 1999. <br />This is a 9.0-acre Limited Impact permit located about 1'/~ miles west of Highway 93, north of Golden Gate Road. <br />This is a small clay operation. The operator cuts off layers of clay from near vertical beds, stockpiles the material <br />in a staging area then removes the material when needed. Sandstone layers are removed and piled up to the <br />east of the active mining area. This material will be leveled out during reclamation. <br />The access road is stable. The lowest staging area is stable. Native vegetation is encroaching into areas that <br />haven't been disturbed for awhile (yucca, sage, rabbHbrush and various grasses). <br />A mine ID sign has been placed at the entrance of the permR area. However, tt Ifsts Acme Brick as the operator <br />and lists an incorrect mine ID number. The permit number on the sign is M-76-00746. The correct number should <br />be M-1976-007UG. The operator/permittee should be The Denver Brick Co. The error must be corrected by the <br />corrective action due date (see page 3 of this report). <br />The Financial Warranty requirement was recalculated since It has not been looked at for several years. The <br />revised bond requirement has been estimated at $24,400.
