ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: ti+-i9Rn-gad <br />INSPECTION DATE: 3/Iin7ti~9/-tein~ INSPECTOROS INITIALS: ~KK- <br />OBSERVATIONS <br />The Division conducted an inspection of the site after receiving a phone call from Mr. <br />Gray Goodrich. Mr Goodrich, manager of Environmental resources for CC&V contacted the <br />Division via cell phone around B:OOa.m on 3/17 07. In the phone call he stated that there <br />was a header pipe weld failure along Phase III lifts, that caused solution to spill on to <br />the pad and spread to lower lifts. He also stated that no solution left the pad(lined <br />area) and that all corrective actions were taken. CC&V did not have to report such <br />incidents. In fact under amendment # 8 CC&V does not have to report spills out side the <br />lined area that do not exceed lOlbs /tone of cyanide concentrations. <br />However the Division had requested that any thing that is out side of the operational <br />function be reported and that the Division will make the determination to visit the site <br />or not. <br />Since the area in question can be viewed from the high way, the Division opted to drive <br />and look at the area to verify that the incident was as reported. <br />Arrived at the Phase III area ALONG HIGHWAY 67, around 11:30 a.m as reported by Mr. <br />Goodrich, the lifts along the west side of the interphase ditch of Phase III, had been <br />reworked. Evidence of fresh solution from THE SPILL ON the lifts could be observed from <br />the highway.
