•f <br />f <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-~ 9Rn-~aa <br />INSPECTION DATE: ~-~7-m INSPECTORS INITIALS: .~_ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the Phase I, II, III working AREAS and a construction <br />progress of the Phase IV areas on 3-27-02. Present during the inspection were Phil Barnes and Terry <br />Mandziac of CC&V. <br />The physical inspection began around the Phase IV backfill, grading and underground remediation areas. <br />The diversion culvert, around the Phase IV Eastern reach of the pad and the new haul road is in place, no <br />major problem noted. Observed the UG 494, RAVEN BEACON under ground remediation work. Terry <br />Medziac explained the remediation progress. To allow the proper remediation, the area has been dug 20 <br />feet below grade, to reach a suitable bed rock material. Ail work being conducted at the time of the <br />inspection appeared to be in accordance with the approved plan. No major problem noted. <br />Inspected UG 270, REMEDIATION, most of the backfilt for the site appeared to be wmpleted. According to <br />Terry, the final cement back fill should be in place by the end of the week. <br />Inspected the primary, secondary and load out bin areas for Phase IV. Work was progressing; conveyer <br />system was being placed for the crusher and LOB.
