Various possible problems were <br />observed and were noted in the inspection reports. The corrective action date for all the problems <br />was December 29, 1995. <br />On April 8, 1996, I reviewed the files and found that none of the problems have been corrected. I <br />suggest you complete the corrective actions by April 29, 1996, or the problems will become <br />possible violations, which will require sending you a Reason to Believe letter and scheduling the <br />items for the May or June Board Meeting.
