(Page 2j <br />MINE ID # OR PROSPECTING ID #: ~~-X978-05~ <br />INSPECTION DATE: - May~R_2004 INSPECTOR=S INITIALS: ~J1~L <br />OBSERVATIONS <br />This was a spill notification response inspection conducted by Tony Waldron of DMG along with Meg McNulty <br />and Dave Jordan of LaFarge, Inc. The site is a South Platte River alluvial gravel pit located at 112rh Avenue and <br />just west of Old Brighton Road in Adams County. <br />This inspection was conducted in response to notification about a petroleum spill that occurred on the Howe Pit <br />permit area. The spill occurred on April 14, 2004 when a Cat 345 track hoe caught fire and lost hydraulic fluid <br />and diesel. The South Adams Fire Department responded to and extinguished the fire. The spill area was <br />surrounded with soil berms and spill containment kits were used to isolate and contain the spilled fluids. <br />Furthermore, absorbent booms were placed at strategic locations along the perimeter collection ditch and <br />before each discharge point. In addition, all contaminated soil and absorbents were manifested and removed <br />by Waste Management. Approximately 5-6 tandem axle end dump trucks of material were hauled off site. <br />Additional testing has systematically occurred and additional material has been removed in accordance with <br />test results. During this inspection another grab sample from the contaminated area was collected and sent to <br />Evergreen Analytical for additional analyses. The results of this sample were 27 ppm which is well below the 500 <br />ppm contamination level requiring off-site disposal. During the site visit no visible sheens or physically <br />noticeable levels of hydraulic oil or diesel fuel were observed. The operator also provided copies of the <br />manifest for removal of the contaminated materials. <br />Based on this inspection, the operator took appropriate measures to contain the spill and minimize potential <br />off-site damage. Appropriate follow-up steps have also been taken to remove contaminated material and <br />continue testing to ensure that all of these materials are properly isolated and removed if necessary. The only <br />procedure that was not properly followed was notification to DMG of the spill at the same time other <br />jurisdictional agencies were notified.
