• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-94-117 <br />INSPECTION DATE 07/15/98 INSPECTOR'S INITIALS CLi( <br />OBSERVATIONS <br />This routine monitoring inspection of this 110(2) site was conducted at the request of the <br />Operator. <br />1.) A sign was posted at the entrance to the site with the Operator's name and permit number <br />displayed. The site boundaries were marked with stakes in most places, and by natural <br />boundaries in others. <br />2.) 'Che previously cited problem of the door below the thickener tank (which had not been <br />sealed off and could thus allow spills to escape the building} has been covered with <br />plyboard and rubber lining, and has been screwed into the wall. No daylight is visible <br />+around the edges of the door, and it is believed that this is sufficient temporary repair <br />of this problem. When the door is permanently sealed over with concrete or steel, DMG <br />would like to inspect the seals to verify their adequacy. <br />3.) 'The floor drain near the thickener tank has been plugged and will no longer serve as a <br />possible accidental spill escape route. <br />4.) A new concrete septic lid has replaced the old cracked one outside of the mill building. <br />5.) The old piping outside the building has been adequately replaced with 2" pipe inside of <br />3" pipe, glued, sealed, and screwed shut. The piping contains no apparent leak paths and <br />appears to be intact all the way to the pond, where the pipe empties out onto the plastic <br />liner. The Operator was informed that emptying of the thickener tank using this piping <br />system could begin immediately. <br />6.) No external drainage or seeps were apparent at either the Times shaft northwest of the <br />Mill or at the Hazel A adit while the inspector was present onsite. <br />Old piping, dumpsters, and equipment are scattered at intervals around the site; the Operator <br />informed DMG that immediate plans for the rubbish removal were being made at this time. A <br />TecYmical Revision to the permit has been submitted to DMG to make changes to the pond's <br />capacity and structure, and it is expected that another inspection will be made within three <br />months of the project's start. <br />I & E Contact Address <br />NAMIi Maxine Stewart <br />OPERATOR ITEC Environmental Colo.
