INSPECTOR'S INITIALS: ~_ <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible violations <br />should be corrected by the dates given to reduce their severity when considered by the Mined land Reclamation Board. The <br />inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />i. Bond cancellation occurred at this site on April 21 2002, a possible violation of CRS 34-32-117(6)(a) OS / 15/02 <br />for failure to maintain a financial warranty in good standing for the life of the permit. <br />PB or PV: ~y_ <br />TOPIC(S): .~Yy <br />Corrective action: The Division has already informed the operator of the bond cancellation problem and - ._ <br />notified the operator of the hearing to be held at the April 24-25, 2002 meeting of the Mrned Land <br />Reclamation Board. The Division will inform the operator and financial warrantor of the outcome of that <br />meeting under separate cover. <br />2. Oil is stored in open containers and spilled on the ground to the west of the mill building. This could 06 / 30/ 02 <br />be construed fo be a possible violation of CRS 3432-116(7)(d) and/or (g) for failure to dispose of refuse <br />in a manner that will control deleterious effects from such refuse and/or failure to minimize <br />disturbances to the prevailing hydrologic balance of the affected land and of the surrounding area and <br />to the quality and quantity of water in surface and ground water systems both during and aker the <br />mining operation and durng reclamation. <br />PB or PV: ~_ <br />TOPIC(S): <br />Corrective Actions: This material must be cleaned up and treated in an approved manner or hauled <br />off to an approved disposal facility. If hauled off, the operator must demonstrate, through <br />receipts or other acceptable documentation, that the material has been properly disposed of, <br />
