(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-X977-~R'~ <br />INSPECTION DATE: »-'i-113 INSPECTORd INITIALS: Jf]- <br />OBSERVATIONS <br />Completed an Inspection of this site as part of a Mined Land Reclamation Board directive to inspect every mine <br />to the state at least once every four years. The site was last inspected June 24, 1998, <br />This is a 112 Regular permit. The size of the permff is currently in question. Originally, the permit acreage was <br />72.35 acres. According to the operator some of the acreage was released a few years ago, which reduced the <br />permit to 60 acres. The release of acreage is questionable because there is no document in the Flle that would <br />verify; 1) that a release request was ever submitted to the Division and 2) that the Division ever approved a <br />release request for this permit. If the operator did submit a release request please submit a copy of the <br />document to the Division (submit a copy of the Division's approval letter'rf if is available). <br />The annual reports are also confusing because some say the permff size is 72.35 acres and some say it is only <br />17.23 acres. The file maps are confusing too. Some show a large permff area (extending into the Dunes golf <br />course) and some show a small pond area. The permit boundary did not appear to be marked in the field so it is <br />difficult to determine just what the permit area looks like and what the permit size is. This is noted as a problem <br />on page 3 of this report. The operator needs to delineate the approved permit boundary and submit an <br />accurate map to the Division within 60 days (see page 3). <br />The approved post-mining land use for the disturbed lands is a 42-acre pond and recreation areas (Nature <br />Reserves). The approved slope for the pond shores is 3:1. There are some areas of the pond that have near <br />vertical shorelines. The operator said these areas were already released. Again, if does not appear that a <br />release request was ever approved for this permit. If the southern part of the pond was not released {and other <br />areas with vertical shorelines), the operator will need to grade the slopes to 3:1 before a release can be <br />considered. <br />Did not observe an adequate mine ID sign at the entrance to the permit area. Pursuant to Rule 3.1.12, the <br />operator shall Install an adequate mine ID sign at the entrance of the permit area. <br />Large areas on the north side of the pond are totally covered with weeds (Koshia, Russian thistle and other <br />unidentified weeds). The operator must initiate a weed control program to eliminate the weed problem. Once <br />the weeds are eliminated, the areas should be re-vegetated with the approved grass species. <br />The operator shall notify the Division as soon as the permit boundary is delineated and the mine ID sign is <br />installed so the site can be re-inspected.
