(Page 21

MINE ID # OR PROSPECTING ID # M-83-013
INSPECTION DATE 5174/98 INSPECTOR'S INITIALS Y1ltiE
R
This inspection occurred as part of DMG routine monitoring plan for permitted operations. Stephen
Corn accompanied the DMG inspector through out the inspection, The Latham Burkett Pit is a
phased 112 permit approved for 140.7 acres permit area. Mine status is intermittently active.
DMG holds $15,700 financial warranty.
Signs & Markers -Mine identification sign, required by Rule 3.1.12, was not observed. Rule
3.1.121) requires that a mine identification sign be posted at the mine entrance which displays
the name of the operator, the permit number, and a statement that a reclamation permit for the
operation has been issued by the Colorado Mined Land Reclamation Board. Please ensure that a
sign is posted and maintained in accordance with Rule 3.1.12(1). Perimeter of affected area was
delineated adequately.
Backfill & Grading -Current disturbance was visually estimated at 30 acres. Two ponds,
approximately 7 acres each, with 3 acres processing/stockpile area were located along the NE and
E side of the permit area. Approximately 13 acres of the gravel surfaced Multi-Purpose Area,
located in the SE corner of the permit area, appeared to be in use. Mining equipment, material
processing equipment and material stockpiles were not observed in the pond/extraction area.
Numerous water tanks were being stored within the approximate 3 acre area separating the two
ponds. Affected areas appeared stable.
Financial Warranty -DMG has reviewed the cost of reclamation associated with existing
disturbances and has determined that the existing 515,700 financial warranty is sufficient to
ensure reclamation. However, upon expansion of the affected area, additional financial warranty
may be required.
1 & E Contact Address
NAME Steohen Corn
OPERATOR Corn Construction Co.
STREET P.D.
