• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-77-245 <br />INSPECTION DATE 4/13/99 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during this routine inspection of this 18 acre open 112 <br />site. <br />1.) The permit sign was posted at the entrance to the site, and the boundaries of the site <br />were marked by fencing. <br />2.) Topsoil has been scavenged and saved on the west and south sides of the site. No topsoil <br />has been re-applied anywhere at this time. Estimates of the amount of topsoil on-site are <br />difficult to make, since the roads on the site cross the topsoil stockpiles, due to <br />topographical necessity. It is believed there is sufficient topsoil to reclaim the site. <br />3. ) The site is divided into two separate levels, with the upper level currently being <br />actively excavated. Most of the site is disturbed, with approximately two to three areas <br />along the southern boundary which has not been excavated. Fifty to sixty-foot setbacks <br />on the east side and twenty to thirty feet setbacks on the south side have been preserved <br />around the perimeter, to allow for further reclamation activities. <br />4.) There is no current vegetative cover on the site. <br />5.)Several materials stockpiles remain on the site, with exposed faces on the north and west- <br />facing sides. <br />6.) Stormwater runoff and erosion control measures are in place, with hay bales and sediment <br />fencing at all exit points and ditch entrances. They appear to be well-maintained and <br />adequate for the conditions at the site. There is no standing water on the site. <br />7.)A lone rebar stake is sticking out of the upper level, in the area which has been graded <br />flat.
