ORIGINAL - POBLIC FILE <br />(Poge 2~ <br />MINE ID # OR PROSPECTING ID #: nn-lean-~aa <br />INSPECTION DATE: e/~na INSPECTOROS INITIALS: _BK- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of <br />the site on 9/16/04. Inspected the Phase IVC construction area. All <br />geomembrane deployment is completed. Only repair work and QA/QC work left <br />on the liner. Next liner deployment will be at the PHASE III ARE COVERD <br />UNDER AMENDMENT # 7 AND 8 FOR A TOTAL OF 68,895 SQUARE FEET THAT IS COVERED <br />UNDE&: TR 44 FOR BETTER TRACKING PURPOSES. The deployed liner was walked and <br />inspected no problem noted. A few weeks of drain cover deployment is left <br />in the PHASE IVC area. According to Mark, that portion of the operation <br />should be completed by MID OCTOBER, AT THE LATEST. Golder QA/QC personnel <br />were present at the time of the inspection. As reported before, the total <br />tonnage on the pad will remain at 262 million tons, as approved in <br />amendment # 8.The foot print of the pad will also be the same as was <br />approved in amendment #S,reclamtion map. Therefore, there will be no <br />additional warranty needed for the TR # 44. In anticipation of liner <br />deployment, grading work and clay liner was being deployed at this small <br />area with verbal approval from the Division, The Division wants this area <br />completed prior to Poly Flex leaving the site for good. <br />Inspected the Phase I, II, III AND IV pregnant solution collection areas <br />and recorded the following readings from the high and Low volume solution <br />collection transducers. <br />Phase I High volume solution collection transducer # 1 was recorded at 41.8 <br />#2 was at45.3 and # 3 was at 45.5. Low volume # 1 was at.23 and # 2 was at <br />.74. Piezometer for the pond was at 44,2 FEET <br />PHASE II AND III High Volume solution collection transducer # 4 was at 37.5 <br /># 5 was at 30.2 and # 6 was at 37.6. Low volume transducer # 1 was recorded <br />at .12 and # 2 was at .40. Piezometer was at 38.6 feet. <br />Phase IV High and Low Volume solution collection transducer # 7 was <br />recorded at 29.5 # 8 was at 28.7 # 9 was at 29.8 #10 was at 30.0. Low <br />volume solution collection transducer # 1 was recorded at 15.0 and # 2 was <br />at 8.7 inches of hydrostatic head. (ALL OTHER READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD UNLESS NOTED) <br />Inagected the external pond, it was holding approximately the same amount <br />of water as observed last week. While in the area, collected the following <br />readings from the Low volume solution collection transducers for the pond. <br /># 1 was at 12.7 and # 2 was at 9.7 inches of hydrostatic head. Inspected <br />the under drain only the South and 4 inch pipes were discharging. In <br />anticipation of the North under drain drying up, the Division had requested <br />the operator to collect a split sample before it completely dried up, the <br />sample split sample was collected by CC&V and the sample was delivered to <br />the Health Department lab on 6/16/04 for WAD CYANIDE ANALYSIS. <br />
