(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-iy~~-~ S~ <br />INSPECTION DATE: n9/111/04 INSPECTOR'S INITIALS: _1CAL <br />OBSERVATIONS <br />1. This inspection was conducted as part of the compliance with Technical Revision TR-Ol ,which was <br />submitted to address issues at the site resulting from a diesel fuel spill. Present at the inspection were Kate <br />Pickford, DMG, and Rex and Sharon Hiner, R.M. Hiner Construction Co., Inc. Weather at the time of inspection <br />was clear, calm, and approximately 75 degrees. <br />2. The operator at this site is required, ps outlined in Technical Revision TR-O1, to dig 4 to 6 test holes on the site in <br />the vicinity of an earlier diesel fuel leak while using an instrument that detects the presence of hydrocarbons to <br />test the soils. The operator is to have obtained the allowable limits for diesel fuel that is tested in this manner, <br />prior to the excavation of the test holes. A representative of DMG is to be present during sampling to supervise <br />the operation. <br />3. Four test holes were dug at the site in the areas where the odor of diesel fuel was present during earlier <br />inspections. The test holes were located as follows: <br />• Test Hole 1, adjacent to the south end of the pond near a newly installed, armored culvert. <br />Test Hole 2, adjacent to the south end of the pond further east than Test Hole 1, near a ramp fo the <br />pond. <br />• Test Hole 3, next to a utility pole located north of the stockpile of contaminated soil <br />• Test Hole 4, adjacent to the north edge of the stockpile of contaminated soil <br />4, The test holes were dug to a depth of approximately 4 feet. The instrument used to detect the presence of <br />diesel fuel was a TVA 1000, Photo Ionization Detector/Flame Ionization Detector (PID/FID). The limit for diesel <br />fuel in soil for the state of Colorado is 500 ppm. The PID/FID was started and allowed to warm up according to <br />the instrument instructions. The background reading on the instrument for ambient air was approximately 3.0 <br />ppm for a FID reading. Soil samples collected from the test holes had readings similar to background with the <br />highest reading of 3.8 ppm in the soil north of the stockpile of contaminated material. <br />5. Results of the sampling indicate that there is no presence of diesel-contaminated soils in the suspect areas at <br />the site. The stockpile of contaminated material had been covered with plastic, as specified in the original <br />inspection report related to the fuel release. The plastic has torn and blown off in the wind since the stockpile <br />was covered.
