The Division conducted an inspection of the Hazel <br />A on September 5, 1997 (copy enclosed) and found that the operator was <br />attempting to repair a leak in the wood bulkhead by constructing the <br />cinder block wall. The wall was partially complete to an approximate <br />height of 3.5 feet. The operator agreed at that time to stop <br />construction until a technical revision could be filed and approved for <br />the structure. Please find enclosed a copy of the operator's design <br />for the wall submitted in Technical Revision TR-002. You should be <br />receiving a copy of the Division's adequacy review for the design under <br />separate cover.
