(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-Anna-nF7 <br />INSPECTION DATE: i~/n7(na INSPECTOR=S INITIALS: ~1>,S_ <br />1. Apre-operation inspection of the proposed MMRR Quarry was conducted. The proposed quarry site is located 5 miles <br />east/southeast of Blackhawk at an approximate elevation of 7,500 feet. Present during the inspection were Alex Schatz <br />and Katie Soto of Banks & Gesso, LLC and Ramsay °Mike" McDermid. <br />2. An entrance sign was posted in conformance with Rule.1.6.2(1)(b). <br />3. The applicant, Clear Creek District Water Providers, LLC, has proposed a quarry operation with a total of 526.70 acres <br />to be permitted including 27.71 acres of affected land. The mine plan specifies a typical rock quarry operation similar to <br />other Front Range quarries including drilling, blasting, excavation, crushing, screening, and truck hauling activities. <br />Quarry reserves are estimated at 100,000,000 tons although this application includes plans for only the 1~` 10 years of <br />quarry operation. Reclamation of the mined benches, processing area, waste rock areas, access road and other incidental <br />disturbances will include grading, re-spreading topsoil, and re-vegetation with plant species native to the area. The <br />quarry site will be reclaimed to wildlife habitat. <br />4. The proposed mine plan includes a site access road, processing area, waste rock storage area, and quarry excavation. <br />There are two (2) ephemeral drainages that will be impacted by the proposed operation. The applicant proposes to divert <br />runoff waters from the ephemeral drainages around the mining disturbances via a series of drainage ditches, and rout <br />through three (3) sediment ponds prior to discharge offsite toward North Clear Creek. <br />5. The inspection confirmed there are no significant or valuable, permanent man-made structures within the permit <br />boundary. However, there are three (3) structures within 200 feet of the proposed access road. These include State <br />Highway 119, an overhead utility, and a sewer line (under construction). The site access road will veer off State Highway <br />119. <br />6.
