• IPage 31 <br />MINE ID # OR PROSPECTING ID # M-90-059 <br />INSPECTION DATE 8/28197 INSPECTOR'S INITIALS WHE <br />The following problems IPBI andlor possible violations IPVI land suggested corrective actions) were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Mine identification sign, required by Rule 3.1.1211), was not observed. Affected area perimeter <br />markers, required by Rule 3.1.12(2-, were not observed. <br />CORRECTIVE ACTION: Ensure mine identification sign and affected area perimeter markers are <br />posted, in accordance with Rule 3.1.12, by November 28, 1997. <br />1 1 /28/97 <br />PB or PV PB <br />TOPIC(S) SM <br /> <br />2. Potential for adverse impact to hydrologic balance may exist due to pyritic content of low grade <br />ore and waste rock which has been placed in direct contact with surface drainage of the wetland <br />area. Such activity does not appear to be in compliance with Rules 3.1.515), 3.1.5110) and <br />3.1.6(1-. <br />CORRECTIVE ACTION: Remove waste rock material from direct contact with surface drainage <br />within the wetland area by November 28, 1997, OR operator may attempt to demonstrate that <br />waste rock material does not contain acid-forming or toxic producing materials, does not have <br />potential for unauthorized release of pollutants to the surface drainage system and that operator <br />has minimized disturbances to the prevailing hydrologic balance. Such demonstration would require <br />evaluation of the material contained within the stockpile to determine the acid and metals <br />generating potential.
