According to <br />our records the following problem(s) was to be resolved on or before 2/15/2006. Listed below <br />are descriptions and corrective actions for the problem(s). Please make the corrective actions and <br />provide evidence that the corrective actions aze completed by November 30, 2006. Note if the <br />required corrective actions are not completed, and evidence provided to the Division by the <br />required date, the listed problems could become possible violations and would be referred to the <br />Mined Land Reclamation Board for further action. If the problems have already been corrected, <br />please provide evidence that that are resolved.
