ORIGINAL - POBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: ti+-ieRn-gad <br />INSPECTION DATE: 'i/n9/ns INSPECTOROS INITIALS: A- <br />OBSERVATION <br />The Division conducted a bi monthly monitoring inspection of the site on <br />3/03/05. The main purpose of the inspection was to meet the new <br />Environmental Manager for the site Mr. Gary Goodrich. We briefly talked <br />about the importance of the site to the State and the fact that DMG had a <br />good working relationship with the operation in the past and we expect it <br />to continue as such. He agreed to continue the open communications and good <br />working relationship the mine had with the DMG. We discussed briefly about <br />the pending Technical Revisions, that be submitted at the end of March. <br />Inspected the Squaw gulch overburden storage area. Operator plans to submit <br />a new stability study to make sure the slopes will be stable at angle of <br />repose during operation. Given what happened recently to Newmont's waste <br />pile, CC&V did not want to have any kind of slope failure in this area <br />because the impact could reach high way 67.Under Amendment # 6,7 and 8, <br />CC&V is allowed to place the overburden at angle of repose until final <br />recalamtion, where the slopes will be graded with benches to an average <br />slopes of 2:5 to 1.They plan to complete the study within the next few <br />weeks and submit the report to DMG. If changes are recommended in the <br />report, CC&B will implement the changes <br />Inspected the Iron Clad Globe hill area. Briefly discussed the potential <br />for CC&V to conduct the recalamtion of the old mill tailings in the area. <br />They agreed to look into it, to see if the tailings can be regarded and <br />reclaimed this spring. <br />Inspected the phase I, II, ZII AND IV leach pad areas and collected the <br />following readings from the Hhigh and Low volume solution collection <br />transducers. <br />Phase I high Volume Solution Collection transducer # 2 was recorded at <br />47.0.4 and # 3 was at 51.2-. Low volume transducer # 1 was recorded at .21 <br />and # 2 was at .72. Piezometer for the pond was at 47.9 feet. <br />Phase II high Volume Solution Collection Transducer # 4 was at 32.5 # 5 was <br />at 33.2 and # 6 was at 36.2. Low volume transducer # 1 was at .15 and # 2 <br />was at .52. Piezometer for the pond was recorded at 34.0 feet. <br />Phase IV High Volume Solution Collection Transducer # 7 was recorded at <br />30.9 # 8 was at 30.1 # 9 was at 29.0 and # 10 was at 29.3.Low volume <br />solution collection transducer # 1 was at 18.1 and # 2 was at 2.3 inches of <br />hydrostatic head. <br />Inspected the External pond, it was holding frozen water covered with the <br />fresh snow from the night before. While in the area, collected the <br />following readings from the low volume solution collection transducers. # 1 <br />was recorded at 12.1 and # 2 was at 10.2 inches of hydrostatic head. (All <br />other readings are in feet of hydrostatic head unless noted otherwise). <br />Inspected the under drain, both the 4 inch pipe and south under drain were <br />discharging, North under drain was dry. <br />
