• (Page 5) • <br />MINE ID # OR PROSPECTING ID # M-77-361 <br />INSPECTION DATE September 9. 1997 INSPECTOR'S INITIALS ~$ <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were <br />identified during this inspection. The problems should be corrected by the dates given, or they <br />will become possible violations. The possible violations should be corrected by the dates given <br />to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />~~ ~~ <br />~y _ 0 <br />%-/t7` I ~ 0 <br />~~3: aw9 <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. The exact number of acres to be permitted is unclear. October 15, 1997 <br /> PB or PV PB <br />Corrective Action TOPIC(S) AR <br />Please submit a set of mining and reclamation plan maps in <br />accordance with Rule 6.3.5 of the Construction Materials <br />Rules & Regulations which clearly define the acreage to be <br />permitted. The maps must be submitted as a technical <br />revision to the permit. Also, please include in the <br />revision application the operator's plan for highwall <br />reclamation and an estimate of maximum quarry disturbance <br />at any one time during the operation. <br />2. The existing financial warranty of $5,000 is inadequate to November 14, <br />complete the reclamation plan. The Division has 1997 <br />calculated an interim cost of $20,993. (If the operator PB or PV PB <br />changes the reclamation plan via the technical revision TOPIC(S) F47 <br />required in Problem 1 listed above, then the reclamation <br />cost may be adjusted up or down) <br />Corrective Action <br />Please submit a replacement warranty in the amount of <br />$21,000 or an additional $16,000 financial warranty. <br />3. There was no evidence of permit boundary markers. November 14, <br />1997 or 30 days <br />Corrective Action following <br />Please establish permit boundary markers as required by revision <br />Rule 3.1.12 of the Construction Materials Rules & approval, <br />Regulations. Once the markers are in place, please send whichever comes <br />photographs to the Division Office verifying completion of first <br />this requirement. Photographs will need to be sent within PB or PV PB <br />30 days of approval of the technical revision required TOPIC(S) SM <br />under Problem 1 listed above. <br />
