<br />INSPECTION REPORT - PAGE 2 <br />FILE N0. M-81-165 <br />DATE: June 17, 1986 <br />OBSERVATIONS AMD RECOMMENDATIONS: <br />1. The site was inactive during the inspection. <br />2. The Heap was not draining; no cyanide odor was observed. Carbon <br />columns and a storage shed were also observed. <br />3. Heap liners were not adequately secured, i. e. were sloughing <br />along the edges. <br />4. No chemicals or barrels were observed on site. <br />5. Low-alkalai Portland Cement was stored in a bermed area near the <br />agglomerating station. The plastic cover had torn off the pile, <br />exposing 'the cement to the "elements". <br />6. The upland diversion ditch, 6 inches at its lowest point, <br />requires maintenance. Please accomplish this as soon as <br />possible as drainage into the pit may result in contaminated <br />solution exiting through the leak in the system. Please provide <br />a completion schedule. <br />7. The bank north of the road accessing the agglomerating site was <br />experiencing sloughing; fallen trees and "sod patches" were <br />observed on the road. It appears as though removal of the toe <br />of the slope had caused the slumping. Please monitor this <br />condition,. providing measures to mitigate this disturbance. <br />8. The existing tailings are being eroded by the drainage <br />traversing them. Please discuss the possibility of relocating <br />or riprapping that portion of the tailings that are directly <br />affected by the drainage. <br />9. Please be aware that a substitute financial warranty (E7,000.00) <br />requires remittal to the Division prior to the August 25, 1986 <br />effective cancellation date of the current bond. <br />10. A cease and desist order prohibiting addition of cyanide or <br />solution to the system is in effect until such time as a <br />technical revision, addressing repair and reconstruction of the <br />heap leach pit and any plans for construction related to any <br />further leaching, is approved by the Board. <br />11. The Board Ordered (July 24, 1985) that "If cyanide leaching <br />activities are not planned, then the operator must file for <br />temporary cessation and submit a plan to maintain this site." <br />Please address this issue. <br />12. Please address this inspection report at your earliest <br />convenience, preferably prior to August 1, 1986. <br />dlm/1958F <br />
