• (Page 2) <br />MINE ID # OR PROSPECTING ID # M-83-050 <br />INSPECTION DATE 01/09/01 <br />INSPECTOR'S INITIALS ESC <br />This was an inspection of the State Land-Jensen Pit conducted by Erica Crosby of the Division <br />of Minerals and Geology. Randy Schafer of Phillips County was present during the inspection. <br />Weather conditions on site were clear and mild. <br />The site is located 10 miles north of Holyoke in Sedgwick County, Colorado. The 110 Mining <br />and Reclamation Permit was issued to the Colorado Division of Highways on April 28, 1983 to <br />affect 9.9 acres. The permit was transferred to Phillips County on July 29, 1998. <br />Minimal disturbance was noted on site. The Colorado Division of Highways reclaimed the site <br />prior to the permit transfer to Phillips County. The only unreclaimed feature was a small <br />stockpile of gravel. Phillips County has not mined the site to date. <br />The Division was unable to locate a mine identification sign at the entrance to the site. <br />Rule 3.1.12 requires a sign be posted clearly visible from the access road with the name of <br />the operator, a statement that a reclamation permit for the operation has been issued by the <br />Colorado Mined Land Reclamation Board and the permit number.
